TERMS AND CONDITIONS OF USE AND RELEASE OF INFORMATION AUTHORIZATION

MyChart: One Place. One Record. One Reason. You.

UConn Health, John Dempsey Hospital, and UConn School of Medicine (collectively referred to as UConn Health) and the physician offices that participate in the UConn Health Electronic Health Information System are pleased to offer access to your medical information via a Web-based version of our computer systems called MyChart.

To request an account, please read and complete this form, indicating that you have read, understand, and agree to these Terms & Conditions of Use and that you authorize the release of information via MyChart at UConn Health.

1) MyChart Access

A. Patient Access.

B. Proxy Access.A proxy is a person who represents a patient. Proxy access to a patient’s MyChart account will be granted to the extent that the individual requesting proxy access has the right to access the patient’s medical information and agrees to do so solely for the benefit of the patient. A MyChart account will be created for both the proxy and the patient.

When a person loses their proxy status, they must immediately inform their provider or the Director of Health Information Management at UConn Health to cancel their MyChart proxy access. The patient may continue to access the account or name a replacement proxy. If the patient is a minor, UConn Health will modify access to MyChart for the proxy and for the patient when the patient turns 13. UConn Health reserves the right to revoke proxy access at any time for any reason.

2) MyChart Use

A. DO NOT USE MYCHART IN AN EMERGENCY OR TO COMMUNICATE URGENT HEALTH MATTERS. In case of an emergency, call 911 immediately, or go to the nearest emergency room.

B. Do not rely on external links for treatment or diagnosis. MyChart may offer links to related medical websites not managed by UConn Health. These link(s) are for your informational purposes only. UConn Health does not endorse and has not verified the accuracy of the information on these websites, and you should not rely on any of the information found on the web sites for purposes of treatment or diagnosis.

C. Allow at least two (2) business days for response to electronic inquiries. UConn Health will use its best efforts to provide timely responses to inquiries made through MyChart. In some cases, the staff that needs to respond to an inquiry or other communication may not be immediately available. Please allow at least two (2) business days for a response.

UConn Health is only able to respond to your communications based on the information you provide. If insufficient information is provided, UConn Health will be unable to provide accurate and reliable services.

D. Consider your e-mail privacy. Patients who use MyChart will be notified via their patient-provided e-mail address when there is new medical information on their MyChart account. Any person with access to your e-mail, such as a spouse or an employer, will be able to see this notification. Although UConn Health will not send private medical information to your e-mail, the notification that new medical information is available on MyChart may be information that you do not want others to know. Take this into consideration when providing an e-mail address.

E. Read and respond to your MyChart messages. You must read and respond to messages in your MyChart Inbox in an appropriate time and manner. Messages that you send via MyChart may be sent to the appropriate department for handling. You acknowledge that UConn Health staff, other than your healthcare provider, may access these messages. UConn Health staff may also access your files as needed to deal with requests or support the operation of MyChart.

F. Contact your treating physician with questions. MyChart provides patients with access to documents such as reports, lab results, and discharge documents that may need additional review and interpretation by their providers. These documents may be viewable on MyChart before your provider has the chance to discuss their content with you. If you have questions about such documents, call your treating physician.

3) Privacy & Security of MyChart Information

A. Privacy. UConn Health is committed to protecting the confidentiality of your medical information. We afford the same degree of confidentiality to medical information stored on MyChart as we give to medical information stored on your UConn Health electronic medical record or in any other medium. We limit UConn Health employees’ access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are used to further safeguard your information. Each time your MyChart is accessed, we will note the date and time of access and identify which medical records were released. We have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use.

B. Security. For other than general information viewing, MyChart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal. Our SSL web server uses authentication and offers the highest level of encryption technology commercially available. You can tell when you have a secure connection by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means that your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to UConn Health by viewing the digital certificate. This certificate verifies the connection between the UConn Health server's public key and the server's identification.

C. Your Responsibilities. UConn Health is not responsible for damages if your acts or omissions result in the violation of the privacy of your medical information. YOU ARE SOLELY RESPONSIBLE FOR MAINTAINING THE CONFIDENTIALITY OF YOUR MyChart USERNAME AND PASSWORD. Take these precautions to keep your MyChart confidential and secure:

1) Do not share your unique identification or MyChart password with anyone.

2) Change your password and notify us immediately if you discover or suspect that someone is accessing your MyChart account without your permission.

3) Keep personal computers used to access MyChart in a safe place, password protected, and updated with antivirus protection.

4) When using a public computer to access MyChart, take safety measures to prevent the next user from accessing your MyChart information (e.g., make sure you are logged out of your e-mail and MyChart account).

4) Availability, Accuracy, and Restrictions

A. Availability. UConn Health strives to keep MyChart convenient and available. MyChart may become unavailable due to system maintenance, back-ups, or technical failures. You are not guaranteed access to MyChart at any particular time and may only access MyChart when it is “available.”

B. Accuracy. Although MyChart displays certain information from your medical record, it does not necessarily display all information found in your medical record. You should contact UConn Health’s Health Information Management Department for an official copy of your medical record. If you think that your health information displayed on MyChart is inaccurate, you can request that UConn Health amend your MyChart health information by messaging your provider within MyChart.

C. Restrictions. You must not use MyChart for fraudulent, harassing, offensive, threatening, or illegal activity. Unauthorized use of MyChart may result in a claim for damages and/or be a criminal offense. If UConn Health suspects that you are using MyChart for fraudulent or inappropriate activity, it has the right, and is the only one who can decide, for any reason, to block access to your account. UConn Health can also block your account if it suspects these types of activities are associated with the account. Your account may be blocked without notifying you in advance. UConn Health has the right to investigate and notify law enforcement of any fraudulent or inappropriate activity associated with your account.

5) Account Cancellation

You can stop using MyChart at any time. Please contact your provider or the Director of Health Information Management at UConn Health to close your account.

6) Copyrights & Trademarks

U.S. and international copyright law and conventions may protect all content included on MyChart and any UConn Health site. This content includes, but is not limited to: text; photographs; graphics; button icons; images; artwork; names; logos; trademarks; service marks; and data. Direct or indirect reproduction of this content, in whole or in part, by any means, is prohibited, except as described below or with the previous written permission of UConn Health.

7) Disclaimer of Warranty

MyChart and all other sites hosted by UConn Health and any content contained in them are provided by UConn Health on as “as is” basis. UConn Health does not represent or guarantee any kind of operation, express or implied, of its sites. UConn Health does not represent or guarantee any content, products and/or services, express or implied, included in these sites. To the fullest extent allowed by applicable law, UConn Health denies all warranties, express or implied. These warranties include, but are not limited to, implied warranties of merchantability, fitness for a particular purpose, title, and/or violations.

8) LIMITATION OF LIABILITY

UCONN HEALTH, ITS PARENT, SUBSIDIARIES, AFFILIATES, AND THEIR RESPECTIVE OFFICERS, TRUSTEES, DIRECTORS, EMPLOYEES, AGENTS, PHYSICIANS AND CONTRACTORS AND ANY OTHER THIRD PARTIES MENTIONED IN THIS DOCUMENT OR ON THIS SITE ARE NEITHER RESPONSIBLE NOR LIABLE FOR ANY DIRECT, INDIRECT, INCIDENTAL, CONSEQUENTIAL, SPECIAL, EXEMPLARY, PUNITIVE, OR OTHER DAMAGES. THESE DAMAGES INCLUDE, WITHOUT LIMITATION, THOSE RESULTING FROM PERSONAL INJURY, LOST PROFITS, LOST DATA, OR BUSINESS INTERRUPTION, ARISING OUT OF OR RELATED IN ANY WAY TO MyChart, THE WEBSITE, SITE-RELATED SERVICES AND PRODUCTS, CONTENT OR INFORMATION CONTAINED WITHIN THE "SITE," AND/OR ANY HYPERLINKED PORTAL OR WEBSITE, WHETHER BASED ON WARRANTY, CONTRACT, TORT, OR ANY OTHER LEGAL THEORY AND WHETHER OR NOT ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. IF YOU ARE NOT HAPPY WITH MyChart, THE WEBSITE, SITE-RELATED SERVICES, AND/OR HYPERLINKED PORTALS OR WEBSITES THERE IS ONLY ONE SOLUTION: STOP USING MyChart, THE WEBSITE, AND/OR THOSE SERVICES.

9)Choice of Law

By using MyChart, you agree that the laws of the State of Connecticut, without regard to principles of conflict of laws, will govern these Terms & Conditions. You further agree that the laws of the State of Connecticut, without regard to principles of conflict of laws, will govern any disagreement of any sort that might arise between you and UConn Health, or any of our affiliates, in relation to your use of MyChart. You agree not to start or prosecute any action in connection with these disagreements other than in the state and federal courts of Connecticut. In addition, you hereby consent to, and waive all defenses of lack of personal jurisdiction and forum non conveniens with respect to location and jurisdiction in the state and federal courts of Connecticut. Any claims related to use of MyChart, the web portal, or these Terms & Conditions must be started within one (1) year of when the cause of action arose, or all rights under such claim shall be lost.

10) Authorization for Release of Information via MyChart

Health Care Representative Proxy Access

What is Proxy?

UConn's MyChart includes a Proxy access feature, where on behalf of the patient,

designated family members or other authorized individuals are granted access to the

patient's select health information through Uconn's MyChart web portal.




We understand you would like to be added as a proxy to a patient's MyChart account. Privacy of protected health care information is important to us. As a proxy, you can act on the behalf of the patient as described below

The proxy will be able to: (1) request appointments for health care services on behalf of the patient, (2) access selected medical information, (3) communicate via UConn's MyChart portal with health care providers about tests, treatments, medications, advice and administrative tasks, (4) view and submit payments via UConn's MyChart portal regarding bills. Please review the Proxy Terms and Conditions to further understand how the proxy can interact with UConn's MyChart portal accounts.

FOR PROXY ACCESS - You must meet ONE of the following criteria:

  • YOU HAVE BEEN INVITED BY THE PATIENT TO BECOME A PROXY ON THEIR ACCOUNT (Patient must sign acknowledgement section below.)
  • YOU ARE THE PARENT OR GUARDIAN OF PATIENT WHO IS A MINOR UNDER THE AGE OF 13 YEARS (level of access will change to Teen Proxy when the patient reaches 13 years.)
  • YOU ARE RECOGNIZED AS THE HEALTHCARE REPRESENTATIVE OF AN INCAPACTITATED PATIENT (this may be determined at the discretion of the UConn Health attending provider.)
  • YOU HAVE LEGAL DOCUMENTATION THAT ALLOWS ACCESS TO THE PATIENT RECORD

Proxy Terms and Conditions

*Please note: UConn's MyChart Proxy Terms are subject to change.

1. The MyChart portal is not a tool to be used in the case of a medical emergency or urgent situation. If an emergency or urgent situation arises, seek appropriate emergency medical service.

2. The MyChart portal is intended as a secure online source of certain confidential medical and billing information. If a user shares his or her MyChart username and password with another person, that person may be able to view health information about the patient.

3. It is the user’s responsibility to select a confidential password, to maintain the password in a secure manner, and to change the password if believed that it it may have been compromised in any way.

4. The MyChart portal contains select medical information from a patient’s medical record and that UConn's MyChart portal is a tool of convenience and does not substitute or reflect the complete contents of the patient’s medical record. The proxy further understands that UConn's MyChart portal contains information from all UConn Health physician offices that use the UConn's MyChart electronic health record system, and that the proxy will be able to access information from all of those physician offices. Such information may include information associated with HIV, mental health and drug and alcohol treatment.

5. Access to UConn's MyChart portal is provided by UConn Health as a convenience to its patients. UConn Health has the right to deactivate access to the patient account or that of the proxy at any time for any reason, including cases where UConn Health reasonably believes that it is not in your best interest to continue to provide UConn's MyChart access to patient or proxy.

6. UConn's MyChart portal is voluntary and not required to be used. As such, UConn Health will not condition treatment upon the patient either signing up for UConn's MyChart portal or authorizing a proxy.

7. The patient may have the ability to revoke proxy access by using UConn's MyChart portal tools. Otherwise, unless UConn's MyChart revokes proxy access (as described in section 9), proxy access will remain in effect until a court order or applicable law revokes it. However, if the proxy has accessed and re-disclosed my information prior to the patient revoking proxy access, such information will no longer be protected by the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Information regarding UConn Health’s obligations and commitments under HIPAA can be found in UConn Health’s Notice of Privacy Practices.

8. UConn's MyChart portal should not be used for frivolous purposes or for purposes unrelated to the care or treatment of the patient.


By accepting these terms and conditions, you acknowledge that you meet the criteria for proxy access for this patient and that you understand the terms and conditions outlined in items 1 through 8.. I am requesting this online access to facilitate more efficient treatment decision making and health related planning.

I understand that this authorization may cover disclosure of CONFIDENTIAL MEDICAL INFORMATION. I specifically authorize release of such information via MyChart. I also understand that I have the right to cancel this release of information through My UConn Health at any time.

I acknowledge and agree that UConn Health is not responsible for my use of the information on my My UConn Health account or on any sites linked from MyChart. I understand that I should ask a physician or other health care provider about my health status or if I have questions about the content on my MyChart account. I also understand that MyChart does not take the place of direct communication with my physician about my health care, such as regarding medications, treatment, and diagnosis that I may need or receive.

I acknowledge and agree that it is my responsibility to periodically check this Terms & Conditions statement for any revisions. My continued use of MyChart after any such revisions shall signify my agreement to abide by them.

This authorization will remain valid unless/until it is revoked.
